Asteron asks many questions on the application form (attached to the relevant Product Disclosure Statement) you must complete when applying for insurance. Your full and accurate answers to these questions are important as they help Asteron determine the cover it can provide.
Many factors are considered (such as your health and occupation) and the information is used to:
a) assess whether Asteron can provide insurance cover, and
b) calculate the cost of your insurance if Asteron has agreed to offer cover.
The premium you pay for your insurance cover depends on the level of risk you present. For instance, some occupations and pastimes present higher risks than others of causing illness or injury.
Similarly, a person's medical history can reveal risk factors that could contribute to the deterioration of their health in the future.
Only questions that are essential to the underwriting of your policy or its ongoing administration are asked.
Some people don’t feel comfortable answering certain questions. To help overcome this difficulty, Asteron will accept the information if it is written out separately, sealed in an envelope marked "Confidential for the attention of the Asteron Chief Medical Officer" and attached securely to the application form. You must sign and date this information.
Your duty of disclosure
The information you provide must be complete and accurate. This is explained fully under "Your duty of disclosure" on the first page of the application form. You should read this carefully before completing the application form.
